Situation of risk management failure that has a base in real life

Assignment 2 is a 3,000-word case study based report. It is weighted at 50% of the module assessment and addresses the module learning outcomes:
• LO2: Critically review leadership theories to improve organisational performance and evaluate the key role that employees and other stakeholders play in an organisation’s approach to risk.
• LO3: Critically review the nature of risk strategy and strategic risk management and develop in respect of supporting those with risk management roles within this.

You are required to identify a situation of risk management failure that has a base in real life. This can be a previously-considered case or a situation you know of. You are required to analyse this situation with regards to the failure, in line with the learning objectives above – what caused the issue, was it people or processes. Details of the actual situation must be included as an appendix in a descriptive form.
Remember to ensure that your analysis is supported by academic literature and is not just a chronological document of the event.
Your assignment is to be presented in report format, include an executive summary, a contents page and a correctly presented bibliography, and be a maximum of 3,000 words (plus appendices).
For clarity, the word count starts from the introduction and ends with the conclusion.
Front pages, bibliography etc are not included in the word count. Any submission exceeding the 10% word count rule is subject to a marking penalty.
You must state the word count on the front page of your assignment. Text should be fully justified and a decimal numbering system used for headings and subheadings.
The following structure and headings are recommended, among others:
1. Introduction, context and objectives
Identification of the issue, explanation of the context of the investigation, the links to people management and organisation development strategies, and the articulation of the objectives.
2. Literature review
A critical review of appropriate literature in relation to the objectives, to reflect current thinking and effective practice; a thematic approach should be adopted which includes a summary.
3. Findings and conclusions
Presentation of the findings, grouped by important issues, that is thematically. Overall conclusions which flow from the analysis and link to the literature review and also to the strategies articulated in Section 1 above.
4. Recommendations and implementation
Recommendations which are feasible, practicable and persuasive, and which recognise barriers or points of resistance and how these might be overcome.
