The appropriate communication tool for this project is a formal memo. The leadership styles I will apply are both authoritative and democratic (Marquis & Huston, 2021).
Memo
TO: All our employees, stakeholders, and Type 2 Diabetic patients
FROM: Lucreshia Jackson, Nurse Manager, Better Health Clinic

SUBJECT: A Change to new ways of monitoring and administrating care for our Type 2 diabetic patients
After careful deliberation and due to the recent rise in numbers of patients with Type 2 diabetes visiting our clinic and therefore affecting our daily clinic operations, I have determined that it is necessary, to begin with, the initial step of implementing the new way of administering care and monitoring our patients with the Type 2 diabetes by implementing a new way of applying telemedicine to do so.
Please be informed that starting from the beginning of October 1, 2022, the following actions are to be implemented in our clinic to address the issue that our clinic faces for the remainder of the year 2021-22 and in the following years to come.
1. Our patients will be required to first communicate with our physicians to confirm whether they are fit to acquire inpatient services in our clinic to avoid congestion in our clinic. Only the patients advised to receive inpatient services by our physiotherapist will be allowed.
2. Our patients must also attend our educational and awareness programs on the new way of administering care and monitoring their conditions, even in remote places. This will help make it easier for our clinic to implement this change as there will be reduced confusion and mix-ups during the implementation of the new change.
3. Our nurses should also be aware that the number of nurses who visit our patients will be reduced from 20 to 10, and they would be required to work in shifts to reduce burnout, affecting their job performance.
4. Our stakeholders are also required to be most involved in this activity as they have the most influence on making it successful. Therefore, support for this change is required, and also they should adapt to this change early and be ready for others to follow their lead.
Therefore everyone should understand that these prudent steps are important to our clinic since they will help impact the increased number of patients with Type 2 diabetes in our clinic. This will result in improved healthcare outcomes, reduced congestion in our clinic, reduced burnout among our nurses and physicians, and increased overall revenue.
