Part I

The Advance Health Care Directive 

Locate a copy of an advanced directive (AD) that complies with the laws of the state in which you work. The organization in which you work should have a copy of an advance directive that is given to patients. If not, download your state’s Advance Directives here http://www.caringinfo.org/i4a/pages/index.cfm?pageid=3289.Links to an external site.

Complete the advance directive for yourself.  Do not turn in your AD with your paper. The AD is for your personal use.

In your paper, Part I:

  1. Identify where you obtained the AD and explain its compliance with state law.
  2. Conduct research (1-2 sources) to learn more about the AD in your state and explain how it works.
  3. Discuss how easy or difficult it was to complete the AD. Your comments should be specific and both objective and For example, when you state your personal feelings, you must relate them to the literature (textbook and research) that discusses this process and the difficulties that many people face when completing an advance directive.

Part II

Physician Orders for Life-Sustaining Treatment (POLST) Form

Part III

Tie the two first sections together by writing a summary and conclusion.

This section should address:

  1. the differences between an Advance Health Care Directive and the POLST,
  2. the RN’s important role in assuring the patient’s right to autonomy in choosing the healthcare interventions the patient does or does not want.

Instructions:
It is anticipated that the initial discussion post should be in the range of 250-300 words. Responseposts to peers have no minimum word requirement but must demonstrate topic knowledge andscholarly engagement with peers. Substantive content is imperative for all posts. All discussionprompt elements for the topic must be addressed. Please proofread your response carefully forgrammar and spelling. Do not upload any attachments unless specified in the instructions. Allposts should be supported by a minimum of one scholarly resource, ideally within the last 5 years.Journals and websites must be cited appropriately. Citations and references must adhere to APAformat.
Classroom Participation:
Students are expected to address the initial discussion question by Wednesday of each week.Participation in the discussion forum requires a minimum of three (3) substantive postings (thisincludes your initial post and posting to two peers) on three (3) different days. Substantive meansthat you add something new to the discussion supported with citation(s) and reference(s), you arenot just agreeing. This is also a time to ask questions or offer information surrounding the topicaddressed by your peers. Personal experience is appropriate for a substantive discussion,however, should be correlated to the literature.
All discussion boards will be evaluated utilizing rubric criteria inclusive of content, analysis,collaboration, writing, and APA. If you fail to post an initial discussion or the initial discussion islate, you will not receive points for content and analysis, you may however post to your peers forpartial credit following the guidelines above.

3. For each article you cited in support of an element of the Plan, provide your thoughts aboutthe strength of the evidence presented in the article(s)

: changes in meds, skincare, counseling, include full prescribing information for anypharmacologic interventions including quantity and number of refills for any new or refilledmedications.
Educational
: information clients need in order to address their health problems. Include followupcare. Anticipatory guidance and counseling.
Consultation/Collaboration
: referrals or consult while in clinic with another provider. If no referralmade was there a possible referral you could make and why? Advance care planning.

There are a lot of important and interesting individuals that have contributed to the history of computers and technology. Learning about some of these will help us to be more appreciative of the history of technology, especially computers.
This assignment will have you selecting an important person from the history of computers (Steve Wozniak is who I chose) and writing up a short report on that person. There is a list of names below that you can select from, or you can suggest another person.
