Setting the scene:
You are the new Registered Nurse for the Humanitarian Entrants Health Service located in Perth
Situation
You have been asked to review the community and develop a health promotion plan, focusing on a health issue (e.g. HIV) relevant to your community. This must be a health issue not already being addressed in your community. You are required to investigate the community further using the McMurray Community Assessment Framework. You are required to choose a target group for your intervention. For example, if you were working as a GP nurse in Wiluna, your target group might be young adults at risk of developing cardiovascular disease. The Health Promotion Project you design must take into account Primary Health Care, health education and health promotion principles. You must consider the social determinants of health and health literacy of your community.
The purpose of this assessment is to introduce you to community assessment frameworks and to explore a health issue from a Primary Health Care (PHC) and health promotion perspective. It also enables you to consider how the characteristics of the target group may influence health promotion strategies.
Guidelines:
Please ensure that you include the following:
- Introduce your chosen health issue, including any relevant epidemiological information, and provide an explanation of why it is an important primary health care issue to address. Research the demographics and level of disadvantage of your community: What is the age, sex and racial or ethnic composition of the community compared to the rest of Western Australia? What is the SEIFA score (if appropriate). Interpret your findings (What does this mean, how is it relevant). Introduce your project and target group. Explain why the program will be important for the target group and wider community. Please note that this must target a community, not a region.
Approximately 400 words
- What is the evidence base for the project? Outline your assessment of the community. What community strengths (assets), resources and risks have been considered when designing the project? Discuss how you have used a SWOT analysis to interpret your research and how this has influenced your project design. Include a brief discussion of any current health promotion programs for this health issue. Approximately 500 words
- Provide a description of the project. What is the level of prevention? (primary or secondary prevention only). Explain the role of the community health nurse implementing the program. Approximately 300 words
- Outline the project goal and three objectives. Objectives should reflect the risk factor you are addressing. (You may use a table here to outline objective and risk factor) Approximately 200 words
- Outline three strategies of the project, provide a rationale for each strategy and indicate which objectives this covers (you can use a table here if you wish) Approximately 500 words
- Provide a brief explanation of how you will evaluate the program. Include pre/post evaluation. Outline your data sources and evaluation questions. This may be in table format (see example below). Approximately 200 words
- Write a conclusion Approximately 100 words
- References must be of a scholarly nature, and may include Grey literature, texts or peer reviewed articles. e.g. AIHW reports, ABS data, Primary Health Network reports, textbook, peer reviewed article. Include a minimum of 15 high quality Of the 15 references, at least 5 must be from peer reviewed journal articles and 5 from Grey literature. These ten references must be less than 6 years old. If you are unsure of the appropriateness of any reference, please ask your tutor for advice. Provide a reference list on a separate page using APAv7 style.
The word count suggestions listed next to the questions are just a guide to assist you, and are not a requirement. Overall the content should be no more than 2200 words +/- 10% (not including reference list, title page or table of contents). The absolute maximum word limit will be 2500 words (this is the 10% variation rounded up to the nearest hundred). You can present the information in tables and/or in paragraphs. Please note that as this is an academic paper, it should be written in the third person.

Note: This must be a new service to this client group, not an adaption/tweaking of an existing health service. You must ensure that there is not already a service in place to meet your identified health need for your chosen community.
Tips for success: Your plan must be realistic and meet the SMART goals – i.e. have a reasonable chance of obtaining funding. Ensure your proposal clearly identifies your nursing role, organisation, location, target group, purpose of your project and the evidence supporting your project.
