Portfolio Assignment Goal Setting

Think of a Long Term Goal that is realistic for you to achieve. You are to create a personal plan of action using the following guidelines:
Title: “Making A Change:___________’s Personal Plan”
Set a Long Term Goal for yourself. Make sure it follows the goal setting strategies described in Chapter 1.
Include 3 things you will need to do to achieve this goal (these are your Short Term Goals used to achieve your Long Term Goal.)
Include 3 reasons why you want this goal (think about your values and traits).
Include 2 barriers that might keep you from completing your Goal.
Include a start date and an end date for your Long Term Goal.
Name three people, groups, or clubs that will support you. BE SPECIFIC; don’t just say friends and family.
Include 2 things that will happen when you accomplish this goal.
List a saying, quote or reward you tell yourself along the way (pep talk).
When complete, upload by the due date.
Layout instructions:
All information needs to be on one sheet. This will be completed on the computer. Click on Word, (blank document). Be sure to save your assignment often
so you will be able to upload it into CANVAS.
Be creative with the layout. Make it pretty, colorful, and personalize it with clip art, pictures or shapes. DO NOT TURN IN BLACK AND WHITE COPY.
