Assignment Objectives

  • Demonstrate the ability to judge behavior (ethical or unethical) of coding by completing Ethical Decision -Making Process Tool based on the facts presented in the case scenario
  • Evaluate the behavior (ethical or unethical) of a coding manager by listing the AHIMA Ethical Coding Standards that would be violated based on the case scenario. Students are mandated to submit their rationale for each selected standard.

Case Scenario    

You are a newly hired medical coding specialist working for a large regional physician group practice. Shortly after being hired, you are informed that the practice is behind on filing claims.  It has been determined that the delay in filling claims is due to delinquent physician documentation. You inform the practice that reimbursements are in jeopardy due to timely filling standards. The coding manager verbally instructs you to take the following actions in relation to the problem:

  • She instructs you not to wait until the physician completes his or her documentation if the encounter occurred more than 15 business days prior to the calendar date
  • She instructs you to use the condition on the billing sheet or triage sheet to abstract your first listed diagnosis code if physician has not documented the first listed diagnosis yet
  • She instructs you to cut and paste from prior EHR notes making it easier for the physician to complete his or her documentation
  • She instructs you to blindly code for the mid-level outpatient Evaluation and Management CPT code if the document is incomplete or missing.

Answer the following questions and be sure to address each part with thoughtfulness and depth (be specific and include examples). Papers should be a minimum of four pages and a maximum of five.

Starting with race, ethnicity and culture, describe your experiences growing up in your family of origin. In your description, be sure to include relevant cultural dimensions as detailed in Cournoyer, Chapter 4, page 92. Remember there are other dimensions, so feel free to include them. Keep in mind that everyone has a culture and ethnicity, even if it is a majority culture. Compare your experiences as a youth with your life today. Describe how your past and present are similar and/or different.
How did your family of origin reflect its ethnic and cultural identity and heritage? How do you reflect it today?
How did adults and children in your family of origin express their thoughts and feelings (e.g. anger, fear, and joy)?
Thinking about your response to question three, how would you characterize your family of origin’s overall communication style (i.e. direct, indirect, clear, masked)?
What cultural aspects do you believe influenced your family communication when you were growing up?
How would you characterize you own communication style today? (i.e. direct, indirect, clear, masked). How has your present communication style been influenced by your family of origin’s communication style?

Paper should be double spaced, size 12 font with 1 inch margins.

Respond to question 1 using the chart below. Questions 2 & 3 should be answered in narrative form.
1. Question #1 at the end of the case study asks how Nucor’s policies and practices reflect different motivational theories (bottom of page 233).
In the following chart, the left-hand column represents a policy or practice adopted by Nucor. Indicate in the right-hand column the correct motivational theory underlying each practice and explain your choice. A particular practice may represent more than one theory. Feel free to add any additional examples you can find. Please explain your answers. Use the following format for your answers: you may use practice 1 (no layoffs) as your 1st example.
