Based on the health and wellness assessment you conducted on your chosen family, complete the following:
Construct a health and wellness plan of care to improve the health status of the family (child, adult, and older adult).
Research available resources in the community that may assist the family with achieving optimal wellness. Include them in your plan (along with contact info for the resources).
At least 1 area of the plan must address a Healthy People 2030 initiative.
The plan must include evidence of interprofessional collaboration in the community to help the family achieve health and wellness.
The plan must provide evidence of consideration of individuals with diverse life experiences, perspectives, and backgrounds.
The plan must address one pandemic-related health risk for the family.
You will be required to deliver the health and wellness plan to the family in Week 4. Faculty must approve the health and wellness plan prior to delivery.
The health and wellness plan must be a minimum of 10 pages (excluding the references). The plan should be written at a 5th-grade level, and bullet points are best (or short paragraphs). Pictures may be used, but pictures may not take up more than 2 of the 10 pages. The plan must address health and wellness
for the entire family (child, adult, and older adult) and will be a total of 10 pages. The health and wellness plan is expected to be professional and appropriate for dissemination to an external body.
