Instructions:
It is anticipated that the initial discussion post should be in the range of 250-300 words. Responseposts to peers have no minimum word requirement but must demonstrate topic knowledge andscholarly engagement with peers. Substantive content is imperative for all posts. All discussionprompt elements for the topic must be addressed. Please proofread your response carefully forgrammar and spelling. Do not upload any attachments unless specified in the instructions. Allposts should be supported by a minimum of one scholarly resource, ideally within the last 5 years.Journals and websites must be cited appropriately. Citations and references must adhere to APAformat.
Classroom Participation:
Students are expected to address the initial discussion question by Wednesday of each week.Participation in the discussion forum requires a minimum of three (3) substantive postings (thisincludes your initial post and posting to two peers) on three (3) different days. Substantive meansthat you add something new to the discussion supported with citation(s) and reference(s), you arenot just agreeing. This is also a time to ask questions or offer information surrounding the topicaddressed by your peers. Personal experience is appropriate for a substantive discussion,however, should be correlated to the literature.
All discussion boards will be evaluated utilizing rubric criteria inclusive of content, analysis,collaboration, writing, and APA. If you fail to post an initial discussion or the initial discussion islate, you will not receive points for content and analysis, you may however post to your peers forpartial credit following the guidelines above.

Consider the following questions in your initial discussion post:

Initial Post
Post a discussion constructed as the ‘P’ (treatment plan) that completes the partial SOAP noteaccessed through the link above.
Include in the discussion:
1. Your treatment “Plan” for the first two diagnoses
(see note below on how to structure the‘P’)
2. Citations for each of the evidence-based practice (EBP) interventions included in your Plan
3. For each article you cited in support of an element of the Plan, provide your thoughts aboutthe strength of the evidence presented in the article(s)

they should be separated out as listed below

Diagnostics
: labs, diagnostics testing – tests that you planned for/ordered during the encounterthat you plan to review/evaluate relative to your workup for the patient’s chief complaint
Therapeutic
: changes in meds, skincare, counseling, include full prescribing information for anypharmacologic interventions including quantity and number of refills for any new or refilledmedications.
Educational
: information clients need in order to address their health problems. Include followupcare. Anticipatory guidance and counseling.
Consultation/Collaboration
: referrals or consult while in clinic with another provider. If no referralmade was there a possible referral you could make and why? Advance care planning.
NOTE:
please input N/A where appropriate for the above 4 categories, do not assume that yourclinical faculty person will know it was not applicable.

Please respond to at least 2 of your peer’s posts with substantive comments using the followingsteps:
Construct a response – ideally one who utilized the same EBP interventions that you did andone who did not.
Substantive comments add to the discussion and provide your fellow students withinformation that will enhance the learning environment.
References and citations should conform to APA standards.
